Estrategias de valoración y manejo terapéutico del dolor postsoperatorio endodóntico: un scoping review

Abstract

El dolor postoperatorio endodóntico es una complicación inesperada con una alta prevalencia y puede verse influenciado por diferentes factores. Actualmente tanto las investigaciones como la práctica clínica se han enfocado en resolver la inflamación vinculada a la sensación de dolor. Lo subjetivo de la sensación de dolor y la información heterogénea sobre este, sumado con la aparición constante de nuevas técnicas, complican la forma de abordarlo. El motivo de la siguiente investigación fue identificar las estrategias de valoración y tratamiento, para el cuidado agudo y preventivo del dolor postoperatorio endodóntico en pacientes adultos de los últimos 10 años. Se realizó una búsqueda sistemática de artículos hasta septiembre del 2021, después se procesó la información mediante EndnoteX9 para luego sintetizar en tablas. Como objetivo secundario se buscó identificar tópicos que requieran más investigación. Se obtuvieron 113 artículos, que abarcaron métodos de evaluación como tratamientos variados, existiendo un predominio por el uso de la escala visual análoga como evaluación y un abordaje terapéutico intraoperatorio, predominando el uso de AINEs como procedimiento más estudiado y la detección de la Crioterapia como una alternativa económica y segura. Se detectaron las siguientes brechas de conocimiento: La evaluación de procedimientos mediante la influencia del dolor en la calidad de vida, el uso de marcadores salivales para la investigación del dolor, junto con estudios clínicos con mayor número de sujetos y tiempos de seguimiento; las cuales podrían ser resueltas en futuras investigaciones a posteriori. // ABSTRACT: Postoperative endodontic pain is an unexpected complication with a high prevalence and can be influenced by different factors. Currently both research and clinical practice have focused on resolving inflammation linked to the sensation of pain. The subjective nature of the sensation of pain and the heterogeneous information about it, added to the constant appearance of new techniques, complicate the way of approaching it. The reason for the following research was to identify assessment and treatment strategies for acute care and Endodontic postoperative pain prevention in adult patients of the last 10 years. A systematic search for articles was carried out until September 2021, then the information was processed using EndnoteX9 and then synthesized in tables. As a secondary objective, we sought to identify topics that require further investigation. 113 articles were obtained, which covered evaluation methods such as varied treatments, with a predominance due to the use of the visual analog scale as evaluation and an intra-therapeutic therapeutic approach, predominating the use of NSAIDs as the most studied procedure and the detection of Cryotherapy as a economical and safe alternative. The following knowledge gaps were detected: The evaluation of procedures through the influence of pain on quality of life, the use of salivary markers for pain research, together with clinical studies with a greater number of subjects and follow-up times, which could be resolved in future investigations.
