Características de identificación y diagnóstico que presenta la solicitud radiográfica de los pacientes atendidos en la unidad de radiología maxilofacial del centro de producción dental del Hospital de Curicó durante el periodo Enero-Junio del año 2015

Abstract

INTRODUCCIÓN: La solicitud radiográfica es una herramienta de comunicación entre el derivante y el radiólogo. Se expresa en un documento con información esencial del paciente, conteniendo además la prescripción de examen y su debida justificación. Existen guías internacionales que ayudan a justificar los exámenes radiográficos en odontología y que sugieren contenidos mínimos en las solicitudes radiográficas. Sin embargo, para el Hospital de Curicó, no existe un protocolo que normalice estos contenidos. OBJETIVO: Evaluar los contenidos de identificación y diagnóstico y el cumplimiento de las prescripciones de examen de las solicitudes radiográficas de los pacientes atendidos en la Unidad de Radiología Maxilofacial del Centro de Producción Dental del Hospital de Curicó durante el periodo enero-junio del año 2015. MATERIALES Y MÉTODOS: Estudio observacional descriptivo, cuantitativo y transversal. Se estudiaron 490 solicitudes radiográficas; sus contenidos de identificación y diagnóstico y el cumplimiento de protocolos local e internacional. CONCLUSIONES: Los dentistas generales emitieron más solicitudes (61,6%). Los contenidos: nombre del paciente, edad del paciente, identificación del clínico que emite la solicitud radiográfica y diagnóstico presuntivo estuvieron presentes en menos de la mitad de los casos (28,7 a 48,7%). El examen más prescrito fue la radiografía periapical (46,9%). Sólo el 4,2% de los casos en radiografías intraorales cumplió registrando el total de contenidos de la solicitud y el 11,3% de los casos cumplió en radiografía extraoral. Otros Hospitales llenaron mejor la solicitud en radiografías extraorales en todos los aspectos evaluados. En radiografías intraorales, los mejores cumplimientos se observaron en el CESFAM y el Hospital de Curicó. Se encontró que el formato dental se llenó significativamente mejor que el formato médico. El grado de cumplimiento protocolo internacional y local fue bajo (24,6% a 28,6%). No se encontró asociación entre cumplimiento de prescripciones de examen con la procedencia de la solicitud radiográfica en los protocolos analizados./ ABSTRACT: INTRODUCTION: The radiology request form is a useful tool for the communication between the clinician and the radiologist. This provides essential information from the patient and regarding the prescription of the examination and its justification. There are several international guidelines which help to justify the X-ray examinations in dentistry which suggest the minimum content of the radiology request form. In the Hospital of Curicó, however, there is no protocol for the contents of the radiology request form. OBJECTIVE: To evaluate the contents of identification and diagnosis, and the fulfilment of the prescription of the X-ray examinations from the patients served in the Unit of Maxillofacial Radiology from the Dental Production Center in the Hospital de Curicó, during the period January-July 2015. MATERIAL AND METHODS: An observational, descriptive, quantitative and cross-sectional study was performed. Contents of identification, diagnosis and the fulfilment of the local and international protocols from 490 radiology request forms were studied. CONCLUSIONS: General dentists were who most emitted radiology request forms (61,6%). The following contents were present in less than half of time: patient`s name, patient`s age, clinician`s identification and presumptive diagnosis (28.7 – 48.7%). The most prescribed examination was the periapical radiography (46.9%). The fulfilment of the contents from the radiology request was reached by 4.2% of the cases in intraoral radiographs and 11.3% in extraoral radiographs. “Other Hospitals” best completed the radiology request forms in all aspects evaluated. Regarding intraoral radiographs, CESFAM and Hospital of Curicó best completed the radiology request forms. The dental request form was found to be significantly better than medical request form. The degree of fulfilment of the international and local protocols was low (24.6% a 28.6%). No association was found between the fulfillment of prescriptions and the institution of origin regarding the analyzed protocols.
