Relación entre desgaste dentario y déficit atencional con hiperactividad en niños entre 7-9 años de la comuna de Doñihue

Abstract

El objeto de este estudio fue 1) Determinar la asociación entre TDAH y bruxismo, 2) Determinar la prevalencia del desgaste dentario en niños con y sin el TDAH y 3) Evaluar la severidad de las facetas de desgaste, en una población de niños de 7-9 años de edad atendidos en los CESFAM de Doñihue y Lo Miranda. La muestra contó con un total de 38 niños que cumplieron con los criterios de inclusión y exclusión. 19 estuvieron diagnosticados con TDAH y 19 sanos. El grupo con TDAH estuvo conformado por 18 niños y 1 niña y el grupo sin TDAH por 10 niñas y 8 niños. Los datos fueron analizados mediante SPSS 15.0, con él se obtuvieron tablas de frecuencia, tablas de contingencia, coeficiente de Kappa, Test Z y Test exacto de Fisher y se utilizó un nivel de significancia del 5%. La hipótesis fue comprobada, los niños diagnosticados con Trastorno de Déficit Atencional e Hiperactividad (TDAH) evidencian mayor prevalencia de facetas de desgaste dentario que niños que no presentan diagnóstico de TDAH. Los resultados mostraron que el 69,8% de las piezas dentarias de los niños con TDAH presentan facetas de desgaste versus 52,2% en el grupo sin TDAH, en relación al tipo de faceta, ambos grupos presentaron más piezas con facetas de desgaste activa, de un total de 536 piezas evaluadas, 277 fueron activas en el grupo con TDAH y 225 en el grupo sin TDAH. En relación a la severidad, se observó que el grupo con TDAH presentó más piezas con severidad grado 2 y 3 que el grupo sin TDAH. El mayor número de piezas, mostró desgaste grado 2 en donde se observa pérdida de tejidos duros menor al 50% del área superficial (exposición de dentina PALABRAS CLAVES: BRUXISMO, TDAH, DESGASTE DENTARIO/ ABSTRACT: The purpose of this study was: 1) to determine the association between Attention Deficit Hyperactivity Disorder (ADHD) and bruxism, 2) to determine the prevalence of tooth wear in children with and without ADHD, and 3) to assess the severity of wear facets, all within a test population of children aged 7-9 years treated in CESFAM of Doñihue and Lo Miranda. The study cohort featured a total of 38 children who met both inclusion and exclusion criteria. Nineteen of the patients were diagnosed with ADHD and nineteen healthy. Among the ADHD subgroup, 18 were male and 1 were female; among the subgroup without ADHD, 8 were male and 10 were female. The data were analyzed using SPSS Statistics software (Version 15.0), with which frequency tables, crosstabs, Kappa coefficient, Z test, and Fishers exact test were obtained. We then tested the hypothesis that those children diagnosed with ADHD have a higher prevalence of tooth wear than those without ADHD. The results showed that among the study participants, 69.8% of teeth exhibited wear among those with ADHD, versus 52.2% of teeth among participants without ADHD, proving our hypothesis at a significance of 5%. In relation to the type of wear, booth groups most commonly presented active tooth wear; out of a total of 536 teeth evaluated, 277 were active in the ADHD groups and 225 in the group without ADHD. In relation to the severity, more of AFHD subgroup participants presented with grade 2 and 3 wear severity in comparison to their cohorts without ADHD. The largest number of teeth showed grade 2 wear, where hard tissue loss (exposure of dentin) is observed in less than 50% of the tooth surface area. KEYWORDS: BRUXISM, ADHD, TOOTH WEAR
